PROCEDURE AND RULES FOR VISITATION:

 

1. Prior to visiting the property, all persons must individually fill out and sign

the Indemnification and Hold Harmless Agreement (Page 2). Page 2 must be

submitted to: Manager, Environmental, Health & Safety, Alabama State Port

Authority, P.O. Box 1588, Mobile, AL 36633-1588. Page 2 must be received

prior to visitation; it may also be delivered in person to the Alabama State Port

Authority, Environmental, Health & Safety office in Mobile. You must keep

this page (Page 1).

 

Authorization must be renewed for each calendar year.

 

2. You must notify the Environmental, Health & Safety Division of the Alabama

State Port Authority (phone: 251-441-7085) prior to your visit. Inform the

personnel of the date and estimated time of your visit, the number of people

involved, and the type of vehicles (s) that will be parked near the property.

 

3. Additionally, if your visit will occur on the weekend, you must also notify the

Port Security unit of the Alabama State Port Authority (251-441-7777) on the

day of the visitation prior to entrance to the facility. Inform the security

personnel of your estimated entry and exit times.

 

4. Visitation will be allowed only on foot during daylight hours. No visitation is

allowed after dark under any circumstances.

 

5. It is your responsibility to park in a safe location off U.S. 90/98 at the

perimeter of the property. Do not block any entrances.

 

6. You must leave this page, (MUDLAKES VISITORS PASS) on the dashboard

of your parked vehicle during your visit.

 

7. Under no circumstances may you leave the established roads.

 

8. This is an active industrial site, not a state park or recreational facility, and

care must be exercised accordingly. You must obey all posted signs. You

may not interfere with any work in progress at the time of your visit, tamper

with any equipment, or remove anything of intrinsic value.

 

Please note that failure to strictly adhere to these rules may result

in the permanent loss of your access to this property, as well as

jeopardize the future access of others. Thank you for your

cooperation and we hope you enjoy your visit.

INDEMNIFICATION AND HOLD HARMLESS AGREEMENT

This authorizes you to visit the Alabama State Port Authority (hereinafter referred to as

ASPA) property located on Blakeley Island for the purpose of bird and nature study for

the calendar year _________. (please complete)

 

By signing the Indemnification and Hold Harmless statement below, please note that you

are assuming all risk in visiting this property, and agree to not hold the ASPA liable for

any incident/damages that may occur during or result from your visit. Furthermore, you

agree to abide by the procedures and rules of visitation listed on Page 1 of this form.

 

The undersigned does hereby agree to indemnify and hold harmless the State of Alabama,

the Alabama State Port Authority and its officers and employees from all damages and

liability for injury to myself or any other person or persons, and injury to or destruction of

property, by reason for any accident or occurrence arising during or resulting from my

visiting the ASPA’s property pursuant to the foregoing authorization.
